Fill out your past medical history- Including uploading any pertinenet lab work, current medication list, allergies, and choose which pharmacy you prefer your medication to be sent to if indicated.

Choose the reason for your visit

Top Notch Vitality treats many common medical issues. Select the reason for your visit from the wide-ranging list of conditions/symptoms we treat.

Answer the targeted questionnaire in relation to the reason for your visit and submit.

Each condition/symptom has its own unique question set to allow for a targeted approach to diagnosis to ensure accuracy and an effective treatment plan.

Questionnaire reviewed by licensed medical provider.

Once the questionnaire is completed a licensed medical provider will review the submission and formulate a diagnosis and care plan. If a prescription is indicated it will be sent to the preferred pharmacy selected in the patient profile.
